Telehealth Informed Consent

Effective date: July 29, 2026

This consent describes telehealth services provided by independent licensed providers through the platform. You will be asked to acknowledge this consent during intake, and it applies to all telehealth interactions on the platform.

1. What Telehealth Is

Telehealth is the delivery of healthcare services using electronic communications where the patient and provider are not in the same physical location. Your care may include asynchronous review of your health questionnaire, secure messaging, phone calls, or video visits, as determined by your provider and required by your state's laws.

2. Expected Benefits and Possible Risks

Benefits include convenient access to licensed providers and timely treatment. Risks include limitations inherent to remote care: your provider cannot perform a hands-on physical examination, technology can fail or be interrupted, and in rare cases delays in evaluation could occur. Your provider may determine telehealth is not appropriate for your situation and recommend in-person care.

3. Your Responsibilities

You agree to provide accurate and complete health information, including all medications, allergies, and conditions; to use medications only as directed; to report side effects promptly; and to seek emergency care (call 911) for any medical emergency rather than using the platform.

4. Alternatives, Withdrawal, and Records

You may withdraw this consent and stop using telehealth services at any time without affecting your right to future care. Alternatives to telehealth include in-person visits with a local provider. Your medical records are maintained confidentially in accordance with HIPAA and state law, and you may request a copy at any time.

5. Financial Disclosure

Program fees are disclosed before you pay and are not billed to insurance. You are responsible for the disclosed fees. A prescription is never guaranteed, and consultation fees compensate clinical review regardless of prescribing outcome where permitted by state law.

Prescriptions are issued only after an online consultation with an independent licensed healthcare provider, who will determine if treatment is appropriate for you. Not all patients will qualify.
